WELL CARE at the European Forum for Primary Care annual Conference

Eurocarers  was pleased to facilitate a workshop at the European Forum for Primary Care Annual Conference on 8 September 2025 dedicated to the WELL CARE project. Since 2005, this Forum has brought together general practitioners and researchers across Europe to promote strong primary care through advocacy, evidence, and knowledge exchange.

During this workshop Claire Champeix introduced the WELL CARE project and shared key findings from the systematic literature review, highlighting effective community-based and primary care interventions from across Europe. The WELL CARE Guide for Implementation, was also introduced.

A selection of identified good practices were explored through an interactive world café format, focusing on strengths, weaknesses, and factors influencing successful implementation. Discussions examined local relevance, barriers and enablers, and the role of primary care professionals in building effective care partnerships.

The discussions revealed strong interest in the project. Key challenges identified included the difficulty of identifying carers and understanding their needs, the lack of shared definitions and language around care, and the need to build trust in health and social services as a foundation for cooperation between informal and professional carers.
